Is it a smart idea to give vaccines to people who are more likely to be immune compromised?Knowing someone will likely have a lower response to infection, also means knowing they will act as a host to incubate the virus in a way to promotes more chance of mutations.

"For example, people with rheumatoid arthritis, multiple sclerosis and other autoimmune diseases commonly receive treatment with anti-CD20 monoclonal antibodies, which ironically destroy cells that make antibodies."https://t.co/VSXxzzxHvP

"Given that both vaccines and therapeutics are aimed at the spike protein, which we saw mutate in our patient, our study raises the worrying possibility that the virus could mutate to outwit our vaccines" https://t.co/gVzgkvI5iH

"Immunocompromised people should be informed about the potential for prolonged viral shedding if they’re infected with SARS-CoV-2 and the importance of self-isolation until they receive a negative test"https://t.co/reT40c6tf6

"Until we know for sure, I think, treating those patients under pretty controlled conditions, as we would somebody who has drug resistant tuberculosis, actually makes sense."https://t.co/KyGlx2xt8U

"The only way to find out how effective the vaccines actually are in immune-compromised people is to wait for data comparing infections between vaccinated and unvaccinated people in different immune-compromised groups, and that takes time."https://t.co/Et0Gtwkeps

"Taken together, these observations indicate that in immunosuppressed patients, inadequate immune control can act as a positive selection pressure to trigger immune-escape mutations and subsequently facilitate the emergence of novel viral variants."https://t.co/mW4icvSsBR

All the symptoms of what I believe is RRMS flair up (That includes losing my legs) exhibits all the ones we see in Covid, even down to not being able to smell.https://t.co/mlqPrhxrhj

Quarantining for 2 weeks.I'm half way in and have "Covid Toes" (Raynaud's), GI issues (Crohn's), a cough (I keep inhaling spit), and sudden fever & fatigue symptoms out of nowhere (multiple sclerosis).I hate the winter.I should buy a thermometer 😅 https://t.co/NrSXbQaj0t

"immunocompromised are more likely to transmit the virus that causes COVID-19 to household contacts. Also, small studies suggest that fully vaccinated immunocompromised people accounted for a large proportion (40–44%) of hospitalized breakthrough cases."https://t.co/mj6vJ9uUaT

"There is not enough data at this time to determine whether immunocompromised people who received the Johnson & Johnson’s Janssen COVID-19 vaccine also have an improved antibody response following an additional dose of the same vaccine." https://t.co/ovxVf2EuRY

"Part of untangling the efficacy of these vaccines in immunocompromised groups will be determining exactly who is protected, or not, and why."https://t.co/2yNc8JAPDA

"There are three mechanisms by which viruses go through evolutionary changes. These are mutations also known as antigenic drift, re-assortment (antigenic shift), and recombination (Shao et al. 2017)."https://t.co/wDXQ95KPpQ

"experts agree that it is essential that current vaccines continue to be administered to as many people as possible. This is because the protection offered by the vaccines is greater than the risk of vaccine escape from potential new variants."https://t.co/GND7c3kxHr

Well, that's not good is it.https://t.co/pfuGJIJhBC https://t.co/O7rbfMGdu0

"It is almost inexplicable why people, when they see the data in front of them, that they don't get vaccinated"https://t.co/qRSYQVmMFNAnd yet... when I read the LITERAL FUCKING CDChttps://t.co/Jqo1r4Cpyu

"There is not enough data at this time to determine whether immunocompromised people who received the Johnson & Johnson’s Janssen COVID-19 vaccine also have an improved antibody response following an additional dose of the same vaccine." https://t.co/ovxVf2EuRY

I am being told that we have overwhelming data, and simultaneously not enough data. Can we make up our minds please?https://t.co/jEI1nCvd42

Did you know that on top of 1 in 5000 people get breakthru cases. Half of those are immune compromised. Studies on people with organ transplants suggest you may need extra shots.https://t.co/51EpLAAtSV

Meanwhile I keep seeing things like this:"Because analyses of these vaccines in people with MS are not available, decisions regarding COVID-19 vaccination and DMT choice should be informed by data and expert consensus" [June 1st, 2021]https://t.co/vX6VftnzEh

"vaccines have undoubtedly conferred widespread protection against SARS-CoV-2 infection worldwide, future studies are needed to identify and mitigate factors [assoc.] with inadequate vaccine response in those with breakthrough infections" [Sept 07, 2021]https://t.co/bZ5yatro4q

"breakthrough infections reported to CDC are an undercount of all [...] infections among fully vaccinated persons, especially of asymptomatic or mild infections. [...] relies on passive and voluntary reporting, and data are not complete or representative."https://t.co/tAuvuKbLEk

Does anyone know where that 1-in-5000 figure comes from? I can only find news articles that seem to be speculation, referencing this news piece https://t.co/o74UzTKJNY

"Emerging data suggest some people with moderately to severely compromised immune systems do not always build the same level of immunity compared to people who are not immunocompromised."https://t.co/yOvnddjrHBhttps://t.co/JOIPkiJYx0

I am being told that I shouldn't worry about heart problems.https://t.co/9LwvWNLboo

Do we have data on Multiple Sclerosis yet?https://t.co/GMa8Fqe8b4

"CDC and its partners are actively monitoring reports of myocarditis and pericarditis after COVID-19 vaccination. Active monitoring includes reviewing data and medical records and evaluating the relationship to COVID-19 vaccination."https://t.co/UGzRDXOp31 https://t.co/cITPDGr4OZ

Did you know Furin cleavage is part of a complex pathway suspected in how Antigen-presenting Cells trigger autoreactivity in Multiple Sclerosis.https://t.co/PJxSPrKNtj

Sure wish we had more data on Multiple Sclerosis. Or animal models that could be used to test with.https://t.co/EEodVK80gE

"these observations support the use of clinical trials to determine whether booster doses to prevent COVID-19 in transplant patients can be incorporated into clinical practice, as they have been for hepatitis B and influenza vaccination" [Sep 2021] https://t.co/4PJs6ktaqn

"Compared with nonusers, patients with CID treated with glucocorticoids and BCDT seem to have lower SARS-CoV-2 vaccine-induced antibody responses. These preliminary findings require confirmation in a larger study." [Aug 31, 2021]https://t.co/wycQkfPEQ9

This is fine.https://t.co/bN3VHpsrLA

The upper left quadrant of that diagram talks about glucocorticoids, which this entire thread dances around. Glucocorticoid is a portmanteau. It means glucose + cortex + steroid. Cortisol (the "stress hormone") is a well known glucocorticoid.https://t.co/fD3ieTtEwK

I am being told that I shouldn't worry about heart problems. https://t.co/tcVtta8BKN

Funny how both Crohn's and Multiple Sclerosis show elevated heart problems and elevated ACE receptor density.Nothing to see here.https://t.co/rCCFz0JDBKNothing worth mentioning https://t.co/SVlJYDIcPV:(

"However, because cardiovascular medical research often focuses on more dramatic interventions, the risk from constipation can be overlooked."https://t.co/HLKl3rtpmWMy stomach is possessed by demons. I am cursed.https://t.co/5EARLlCORH

Booster tests in the main study group in the trial rain by Pfizer weren't tested on immune compromised people...Why?https://t.co/Gbu5j2ylAU https://t.co/mSj09fmlPa

Here, @EricTopol cites that study as 'Incontrovertible Evidence for Booster Benefit"Yes. In healthy people. How long to wait for studies on immune compromised? Last time it took months before John Hopkins even started testing it on people like me.https://t.co/sS7W4ghSf8 https://t.co/VaMhqtv6a4

He's right to point out the oddities here, but @EricTopol neglects another possibility... The initial datum are largely from healthy people. We see an obvious divergence here when the vaccines and boosters are applied to the population in the wild. https://t.co/Y2d6uRAAAh

I will keep citing the FUCKING CDC about this point... but we'll never change how we test this stuff, will we?https://t.co/qfMBzS3Gta

Can we... Uh... stop fucking up the freaking basics?https://t.co/HP0pyLa99i

Can we talk about predictable outcomes of telling the western world that the vaccines are 100% effective, while neglecting the fact that we had to wait for cancer patients and people on immune modifying drugs to even be trailed against? https://t.co/D95KdkLwxd

Cull the sick, this shits twisted.https://t.co/mVKsaKINfc

I'd love to have a discussion about the nature of what makes a vaccine avoid viral mutations related to resistance. An immune compromised person violates these 'generalities'. and the current vaccines seem to violate the 'multiple targets' bit.https://t.co/qMX2w3Tqmn https://t.co/g3UQ1RDyJc

Immune escape... what a funny word. Glows red to me.Shinigami. Blood. Anger. Emotional synesthetic memory encoding from when anger makes me literally see red due to blood pressure infusing into my vision.https://t.co/et1vnThG6p

"Taken together, these observations indicate that in immunosuppressed patients, inadequate immune control can act as a positive selection pressure to trigger immune-escape mutations and subsequently facilitate the emergence of novel viral variants."https://t.co/mW4icvSsBR

"finding of SARS-CoV-2 viral shedding in 4% of COVID-19 patients seven months after infection suggests that a small minority of patients may have gastrointestinal symptoms related to infections months after an acute illness."https://t.co/jEXzkkryMTWonder if we'll find out why.

Sure wish we knew what was going on here.https://t.co/ZNfZ8Ma9Gh

LMAO, someone has been probably carrying covid for 2 years and it's leaking out in their shit. https://t.co/C0tBJVP6S8https://t.co/yD1DEY4dmC

GF's mom got a positive Covid Antigen Test and now GF has a fever and is coughing. I got a sore throat. So I guess I'm making a thread on bluesky about how we are treating it.I am not a doctor.https://t.co/sddD0Vo32L
